Number of Money Follows the Person Transitions Since Inception, 2015

The Money Follows the Person program is a federal grant to help states "rebalance" their Medicaid LTSS (long term support and services) towards HCBS (home and community-based services) and away from institutional care. It provides additional federal money to states to help them transition persons with disabilities from institutions back into the home and community. By 2015, 75,150 persons with disabilities have been successfully transitioned back into the home and community through this program. Currently, 44 states and DC have MFP programs and the above map shows the number of persons transitioned out of institutions by this program by state. Texas was the most successful state for such transitions with 11,433 transitions under the program while in the same period, South Carolina transitioned the least with 68 persons.

Number of Money Follows the Person Transitions Since Inception Color coded map of the United States.  States have been grouped into four quartile ranges. The first quartile ranges from 68 to 300 and includes the states Alabama, Colorado, Washington DC, Maine, Minnesota, Montana, Nevada, Rhode Island, South Carolina, South Dakota, Vermont & West Virginia.  The second quartile ranges from 301 to 733 and includes the states Delaware, Hawaii, Idaho, Iowa, Kentucky, Mississippi, Nebraska, New Hampshire, North Dakota & Oregon.  The third quartile ranges from 734 to 2,499 and includes the states Arkansas, Georgia, Indiana, Kansas, Louisiana, Massachusetts, Missouri, New Jersey, New York, North Carolina, Oklahoma, Tennessee, Virginia & Wisconsin.  The fourth quartile ranges from 2,500 to 11,433 and includes the states California, Connecticut, Illinois, Maryland, Michigan, Ohio, Pennsylvania, Texas, United States & Washington.
